Q) LOST MY MEDICAID CARD MOVED TO PA AND DO NOT KNOW HOW TO GET A NEW ONE. I WENT TO THE OFFICE OF MEDICAID AND I HAVE LOST ALL MY ID THEY WILL NOT HELP ME PLS ADVISE.
A) I’m sorry to hear that you lost your Medicaid card and moved to PA. I understand that it can be frustrating and difficult to get a new card without any ID.
Here are some things you can do to get a new Medicaid card:
- Contact your old state’s Medicaid office. They may be able to send you a replacement card or help you get a new one.
- Apply for Medicaid in PA. You will need to provide proof of your identity and residency, but you can do this without a current ID.
- Visit your local social services office. They may be able to help you get a new Medicaid card.
Here are some documents you can use to prove your identity and residency:
- Birth certificate
- Social Security card
- Driver’s license
- State ID card
- School ID card
- Pay stub
- Bank statement
- Utility bill
- Lease agreement
- Mortgage statement
If you are unable to provide any of these documents, you may be able to get a letter from a trusted friend or family member who can vouch for your identity and residency.
I hope this information is helpful. Please let me know if you have any other questions.
