I don’t have access to send you physical mail. However, you can obtain an application for food stamps and Medicaid in New York by visiting the website of the New York State Department of Health at https://www.health.ny.gov/health_care/medicaid/apply/ or by contacting the New York State Office of Temporary and Disability Assistance at 1-800-342-3009. You may also be able to download the application form from their website and print it out to fill in manually.
