However, you can obtain an application for food stamps and Medicaid in New York by visiting the website of the New York State Department of Health at https://www.health.ny.gov/health_care/medicaid/apply/ or by contacting the New York State Office of Temporary and Disability Assistance at 1-800-342-3009. You may also be able to download the application form from their website and print it out to fill in manually.
