If you need a replacement card for your Medicare or Medicaid, you can request a new one through the appropriate agency.
To get a replacement Medicare card, you can:
Visit the Social Security Administration’s website at https://www.ssa.gov/medicare/replace-your-medicare-card.html
Scroll down and click on the “Request a Replacement Card Online” button, then follow the instructions on the screen.
If you prefer to request a replacement card by phone, you can call the Social Security Administration at 1-800-772-1213.
To get a replacement Medicaid card, you should contact your state’s Medicaid agency. In New Mexico, you can contact the New Mexico Human Services Department at 1-800-283-4465 for assistance with getting a replacement Medicaid card.
