I am attempting to update last name due to change per Divorce Decree. With Medicaid

Q) I am attempting to update last name due to change per Divorce Decree.
I have finished this process with S.S., Medicare and am trying to do so with Medicaid.
I was told to call my local office which was not helpful though I did use the option to update such information.

A) I’m sorry to hear that you’re having trouble updating your last name with Medicaid. Here are a few steps you can take:

  1. Gather your divorce decree: Make sure you have a copy of your divorce decree that shows the change of name. This is the most important document you’ll need to update your last name with Medicaid.
  2. Contact your local Medicaid office: Try calling your local Medicaid office again and explain that you need to update your last name due to a divorce. Be sure to have your new name, your Medicaid identification number, and your divorce decree handy. If the person you speak to is unable to help you, ask to speak to a supervisor.
  3. Submit a written request: If you’re not able to update your last name over the phone, you can submit a written request to your local Medicaid office. Be sure to include a copy of your divorce decree and any other required documents.
  4. Follow up: After you submit your request, follow up with your local Medicaid office to ensure that your name has been updated. If you continue to have trouble, you can contact your state’s Medicaid ombudsman for assistance.

Remember that updating your last name with Medicaid may take some time, so be patient and persistent in your efforts.
