My mother who was on IN Medicaid passed away on June 24, 2023. She had a Qualified Income Account, which had $6.99 remaining in the account when she passed. The bank closed out the account and wrote a remittance check for the above amount that was to be sent to Medicaid. Where do I mail that remittance check?

Please make sure to include the following information on the check:
The deceased beneficiary’s name
The date of death
The amount of the check
The reason for the check (e.g., Qualified Income Account closing)

Here are some additional things to keep in mind when mailing the remittance check:
The check should be made payable to “Indiana Medicaid”.
The check should be in U.S. dollars.
